Poka-Yoke is a commonsense approach to defect elimination. Traditional quality systems detect defects after they occur. An operator makes an error which becomes a defect. At some point after the defect happens an inspection occurs and the defect is detected. Subsequently, people are notified of the defect and corrective action (scrap or rework) is initiated. Poka-Yoke and Process Improvement are important and effective. This training module is dedicated to providing the tools required in order to understand all the options for improvements. Anything you do, can be improved. The History of Mistake-Proofing (Poka-Yoke)Mistake-proofing was developed in 1961 by Dr. Shigeo Shingo, the late Japanese quality guru. At that time, Shingo was visiting an electrical parts factory in Japan. The tour was conducted by a plant manager. Shingo and the plant manager were observing a woman assembling switches. Her job was to insert two springs into the switch box. This kind of routine, repetitive procedure lends itself to inadvertent errors. In fact, as they watched, the worker assembled a switch with only one spring. Seeing this, the manager yelled at the woman asking her how she could be so careless. Dr. Shingo reprimanded the manager, informing him inadvertent errors were a part of the human condition. Dr. Shingo left and returned from the plant cafeteria with a rice bowl. He told the worker from now on the first operation in the switch assembly process would be to take two springs from the box and place them in the rice bowl. Before she passed the switch to the next production associate she should look at the bowl. Then there would never be any doubt that the switch contained the correct number of springs. That troublesome step in the process of switch assembly was mistake-proofed. In the following years, Shingo refined the concept of mistake-proofing and introduced the idea of zero quality control. Poka-Yoke may be the single best method to eliminate or substantially reduce defects. Originally designed for use in manufacturing, it has enormous potential for government and service industries as well. It is a simple, straight forward concept which prevents errors from becoming defects using inexpensive devices to mistake-proof the process.
